In this pivotal role, you will play a crucial part in ensuring the highest standard of care for our residents while leading our care planning team to success.Salary: $42-$46 hourCompensation and Benefits:Comprehensive Benefits Package: Including generous paid time off, medical/dental coverage, and a 401(k) plan.Responsibilities:Quality Care Oversight: Ensure the delivery of high-quality care to our long-term care residents.Comprehensive Assessment: Conduct thorough initial interviews and assessments for new residents.Care Planning: Develop comprehensive care plans tailored to individual needs, encompassing both short-term and long-term improvement goals.Interdisciplinary Collaboration: Facilitate productive meetings with multidisciplinary treatment teams, caregivers, and families to ensure coordinated care.Community Engagement: Collaborate with community specialists to achieve optimal outcomes for our residents.Leadership and Education: Lead by example in Nursing management team meetings, provide effective communication with all levels of Nursing staff, and conduct informative in-service education sessions on MDS, RAPS, and Triggers.Adherence to Schedule: Ensure adherence to the MDS/Care Planning schedule for non-Medicare residents.Qualifications:Florida RN License: Valid and in good standing.Skilled Nursing Experience: Minimum of 2 years of experience in a Skilled Nursing Facility.Expertise: Knowledge of MDS, Medicare, and the care-planning process.If you are passionate about enriching the lives of our residents and embody values such as safety, care, professionalism, efficiency, strong team orientation, and exceptional communication, we encourage you to apply!Join us at Boca Circle Rehabilitation Center and be part of a team committed to excellence in care.IND123by Jobble
