for the role of Licensed Practical Nurse (LPN) in our WillowBrooke
Court (Skilled Nursing) neighborhood.In this role, you will provide
prescribed medical treatment and personal care services to our
residents in skilled nursing. Participates in the supervision of
nursing assistants, and coordinates the provision of resident care
according to the care plans.The ideal candidate will meet the
following requirements:* Current state Licensed Practical Nurse
(LPN) license* Current or eligible for certification in
Cardiopulmonary Resuscitation (CPR)* Minimum of one year experience

Requirements:Participates in the supervision of nursing assistants,
and coordinates the provision of resident care according to the
care plans. 2. Takes an active role in direct patient care. 3.
Participates in the formulation of individualized nursing care
plans. 4. In conjunction with Registered Nurse prepares daily
assignment for nursing assistants. 5. Accompanies the physician on
rounds to ensure the exchange of pertinent information. 6. Ensures
that physician's orders are transcribed correctly. 7. Administers
medication and performs treatments (i.e. -- dresses wounds, gives
enemas, douches, applies compresses, ice bags and hot water
bottles) as ordered by the physician and in accordance with ACTS
policies and procedures. 8. Identifies potential safety hazards and
takes appropriate action to correct the situation. 9. Maintains
accurate resident care records and documents pertinent data. 10.
Attends and participates in resident-care meetings, staff meetings
and all mandated in-services. 11. Takes and records residents'
vital signs. 12. Observes residents and reports adverse reactions
to medication or treatment to nursing personnel in charge. 13.
Collects samples, such as urine, stool, and sputum, from residents
for testing and performs blood glucose monitoring. 14. Monitors
meal times and food delivery for prescribed diet and assists in
feeding residents when needed. 15. Demonstrates, understands and
uses medical equipment (i.e. -- catheters, tracheotomy tubes,
feeding pumps and oxygen suppliers. 16. Assists nursing assistants
in the performance of their essential job duties when needed. 17.
Reports changes in the Resident's condition, complaints or concerns
immediately to the Director of Nursing or RN supervisor. 18.
Answers signal lights, bells or intercom system to determine
resident's needs. 19. Follows established guidelines and performs
duties per the Nursing Policy and Procedure Manual. Licensed
