The Care Manager plays a vital role within our interdisciplinary
team, focusing on assessing, planning, advocating, and coordinating
patient care from the moment of admission through to discharge.
This position ensures a smooth transition after hospitalization,
aligning with our organization's mission, values, and service
standards. Our Care Management model emphasizes effective
transition planning and oversight of length of stay, all aimed at
enhancing patient experience, safety, and quality of care through
the use of performance metrics and best practice adoption. Position
Overview: Qualifications: Essential Education: Must be a graduate
from an accredited School of Nursing. A Bachelor’s Degree in
Science of Nursing is required. Essential Experience: At least 1
year of experience in an acute care hospital environment. Licensure
Requirements: Must hold a current RN License in the state of
employment. Certifications Required: BLS (Basic Life Support)
certification is required upon hire or within 90 days. Preferred
Certification: Certification in Case Management is preferred.
Preferred Experience: 2 years of experience in an acute care
hospital setting. Experience in Care Management or Utilization
