our family. The Primary Care Physician (PCP) within our
organization is expected to: Be accountable for patient outcomes:
The PCP takes responsibility for delivering strong clinical care
and cost-effective solutions for a panel of up to 450 patients.
They recognize the significant impact they can have on patient
outcomes by fostering trusting relationships and encouraging
positive behavior changes. Act as a health coach: Rather than
merely addressing illness, the PCP helps patients establish both
short- and long-term health objectives, collaborates with them to
achieve these goals, and regularly follows up to ensure progress
toward improved health. Simplify actions: The PCP prioritizes tasks
effectively to make behavior changes more achievable for patients
and guides their care teams toward meeting performance targets. As
a value-driven, outcomes-focused organization, we regularly assess
various metrics for each PCP's patient panel to support their
success and help them achieve partnership status. These metrics
include patient admissions per thousand, appointment slot
utilization (18-21 slots daily), CGCAHPS (patient experience),
clinical gap closures, and medical cost measures. Each PCP will
have specific goals related to these metrics and will collaborate
with center and market leadership, as well as care teams, to meet
these targets. Fostering a positive culture of love,
accountability, and passion is crucial, and PCPs are expected to
lead this effort within their centers. The PCP must demonstrate the
ability to work both independently and collaboratively with other
healthcare professionals. They will closely coordinate with
relevant managers and medical directors to ensure adherence to
guidelines and participate in risk and quality management
initiatives, clinical meetings, and other gatherings that promote
patient health and organizational objectives. Compliance with
departmental goals, performance standards, regulatory requirements,
quality patient care standards, and established policies is
essential. KEY JOB DUTIES AND RESPONSIBILITIES: The PCP
independently manages care for patients with both acute and chronic
conditions, particularly in older adults. They take full
responsibility for patient care and outcomes, seeking specialist
consultations when necessary while remaining engaged in the
patient's comprehensive care. The PCP is expected to communicate
with the hospitalist whenever one of their patients is
hospitalized, regardless of the hospitalist's affiliation.
Responsibilities include assessment, diagnosis, treatment,
management, education, health promotion, care coordination, and
documentation for patients with complex health needs. The PCP leads
a care team that includes a medical assistant, care facilitator,
and care coordinator for patients who can visit the office. For
patients unable to attend the office—such as those in hospitals,
skilled nursing facilities, long-term care, or homebound—the PCP
collaborates with the transitional care team and other available
resources. The PCP plays an active role in managing their center
and may cover for other providers as needed. They are also expected
to assist in recruiting patients and additional providers for the
organization. They supervise, collaborate with, and are accountable
for the actions of Advanced Practice Practitioners (APPs) within
their practice. Other duties may be assigned at the discretion of
management. KNOWLEDGE, SKILLS, AND ABILITIES: Competencies for
Success Availability and Accessibility: PCPs should be approachable
and available to their patients, encouraging trust by being open to
walk-in visits and responding to calls and messages promptly.
Service Orientation: PCPs deliver care that they would want for
their own family members, ensuring a compassionate approach at
every interaction. Evidence-Based Medicine: PCPs stay informed
about evidence-based practices while acknowledging the significant
role of lifestyle and nutrition in patient health outcomes.
Physician Leadership: Continuous development of leadership skills
is essential for the benefit of patients, teams, and the
organization. Quality: Commitment to providing the highest quality
of care is paramount, requiring collaboration with care teams to
achieve excellent outcomes and ongoing improvements in practice.
Influence: PCPs must effectively and compassionately motivate their
patients, teams, and themselves to achieve optimal results.
Self-Care: PCPs can best care for their patients when they
prioritize their own physical, mental, social, and spiritual
well-being. Proficiency in Microsoft Office Suite, including Word,
Excel, PowerPoint, and Outlook, as well as other relevant software.
Willingness to travel locally as needed, and occasionally
regionally or nationally for training or recruitment purposes.
Fluency in English, both verbal and written; some positions may
require proficiency in additional languages, which will be
specified during the application process. This role requires
independent judgment and decision-making. SALARY RANGE: $221,141 -
