The primary role of the Medical Social Worker (SW) is to assess the
psychosocial needs of patients and provide solution-focused care
management to address issues that impede their ability to adhere to
their medical care plan. The SW is an active member of the
Interdisciplinary care team (IDT). Social work services will be
provided in home or via telemedicine as determined by state
regulations and/or market need.
If you are located in Boca or Palm Beach County, FL, you will have
the flexibility to work remotely as you take on some tough
challenges. 60/40 - 40 home / 60 in the field.

If the hired individual resides in Florida (office based or
telecommuting) this position requires the AHCA Level II background
check (fingerprinting) by the State of Florida for all clinicians
that have direct face to face contact with members OR employees who
will have access to confidential patient data and will require
renewal every five years.
