JOB CODE: (350120) RN/Home Health LOCATION: Central Broward and Martin County (FL) Two positions available with up to $3,000 sign on bonus: 1. Full Time RN position in Central Broward Boca Raton (FL) 2. Full Time RN position in Martin County including West Palm Beach, Palm Beach Gardens, Jupiter, Hobe Sound, Stuart, Palm City, Jensen Beach, Port St. Lucie This is a Per Visit pay model and requires traveling from patients to patients home. Seeking experienced Home Health RNs to join a team of caregivers who strive to help patients recover safely and independently in the environment they feel most comfortable in their home. Caregivers work with the patients physician and provide care planned specifically for the patients needs, designed to help them take control of their treatment, prevent future declines in their health and improve their safety at home. Clinicians visit the patients home to deliver customized, physician-prescribed care after a hospital stay, illness, or injury to help them rehabilitate and achieve an optimal level of independence or to proactively manage chronic conditions to help prevent avoidable hospitalizations. The Home Health RN works full time to administer nursing care for clients in their place of residence, coordinate care with the interdisciplinary team, clients, and their families, and the referring agency. Must be a team player who works well with the patients in their homes, is dependable and follows the Companys mission of putting our patients first. Patient Population: Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with patients in all populations. Supervision: LVN/LPNs and CNAs Essential Functions: Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health clients. Completes assessments at appropriate time points, including OASIS. Regularly re-evaluate the clients nursing needs and evaluates the outcomes of care. Develop, initiate, and revise the Plan of Care (POC) as necessary to ensure quality and continuity of care. Initiates appropriate preventative and rehabilitative nursing procedures. Plans for discharge of the client from services. Provide effective communication to clients, their families, team members, and other health care professionals. Regular participation in Case Conference. Apply concepts of infection control and universal precaution in coordination/performing client cares activities to protect both client and staff (OSHA). Coordinate services and informs the physician and other personnel of changes in the clients condition and needs. Monitor assigned cases to ensure compliance with requirements of third party payers. Complete appropriate documentation in a timely manner to assure compliance with company policy. Supervise, teach and provide clinical direction to other nursing personnel (ex. LVN/LPN, HHA, CNA, etc.). Perform on-call responsibilities and provide on-call services to clients and their families as assigned. Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency. Meet mandatory continuing education requirements of the agency/licensing board. QUALIFICATIONS: One (1) year of experience as an RN in a clinical care setting required; prefer one (1) year of Home Health experience Nursing skills as defined as generally accepted standards of practice Excellent interpersonal skills and ability to communicate effectively Demonstrates proven decision making skills If required to make visits, Hepatitis profile Must read, write and comprehend English Completion of an accredited Registered Nursing Program BSN preferred Current RN license for the state you are applying or a compact license which includes the state you are applying Understands that this position pays using the Per Visit pay model Understands that this position requires traveling from patients to patients home Current drivers license in good standing. It is the responsibility of the employee to renew their drivers license before it expires in order to continue employment. Reliable transportation and auto liability insurance Physical Requirements: Prolonged standing/walking required. The ability to lift/transfer up to 50 lbs. Ability to meet deadlines and patient needs, make quick decisions and allocate resources under stressful conditions. Meet patient/family individualized psycho social needs. Requires hand-eye coordination and manual dexterity. Environmental/Working Conditions: Works in patients home in various conditions. Possible exposure to blood borne pathogens, bodily fluids and infectious diseases. Some exposure to unpleasant weather; PRN emergency call. Ability to travel locally and work a flexible schedule. Machinery/Tools/Equipment Requirements: Reliable transportation and auto liability insurance Manual blood pressure cuff Stethoscope Thermometer Ongoing employment as an RN requires maintenance of a valid RN license in the state which the nurse practices and a valid CPR certificate at all times.
